The org's reusable intake forms, newest first, each with a usageCount of the contents currently attaching it.

Create an intake form

POST/intake-forms

A named set of enrollment questions (max 10). Attach it to a content with PATCH /contents/{id} and its intakeFormId field.

Update an intake form

PATCH/intake-forms/{id}

Rename the form and/or replace its questions. The reference is live — changes apply everywhere the form is attached. Keep question ids stable so existing answers stay linked.

Delete an intake form

DELETE/intake-forms/{id}

Contents pointing at the form get their intakeFormId set to null and fall back to their own inline questions. Answers already collected are kept.
